If necessary, once you submit this form a request for confirmation will be sent to the email address we have on file for the account. You must follow the instructions in this email, after which your account, including all files and information, will be completely removed from the system. This data cannot be retrieved after the account is closed, so please make sure that you have a local copy of your files before submitting this request.
